辦理機電證需要什么資料

三、進(jìn)口用戶所需提交的紙質(zhì)材料

（一）申請《機電產(chǎn)品進(jìn)口自動許可證》，應(yīng)當(dāng)提供以下材料：

1.機電產(chǎn)品進(jìn)口申請表蓋章原件；1 Mechanical and electrical products import application form seal original;

2.進(jìn)口用戶的營業(yè)執(zhí)照復(fù)印件；

2 copy of the business license of the import user;

3.貨物進(jìn)口合同復(fù)印件；3 copy of the goods import contract;

4.屬于委托代理進(jìn)口的，應(yīng)當(dāng)提交委托代理進(jìn)口協(xié)議；

4 where the principal agent is imported, it shall submit the import agreement of the principal agent;

5.如屬于下列情況的，還應(yīng)提供以下材料：

（1）進(jìn)口醫(yī)療器械類機電產(chǎn)品的，應(yīng)提供有效的進(jìn)口產(chǎn)品的醫(yī)療器械注冊證和進(jìn)口單位的醫(yī)療器械經(jīng)營許可證（復(fù)印件）；

（2）進(jìn)口數(shù)控機床類機電產(chǎn)品的，應(yīng)提供《進(jìn)口數(shù)控機床技術(shù)參數(shù)表》（蓋章原件）；

（3）進(jìn)口煙草設(shè)備類機電產(chǎn)品的，進(jìn)口設(shè)備須在國家煙草專賣局編制的年度計劃表中且申請表須加蓋煙草專賣局機電辦轉(zhuǎn)報章；

（4）進(jìn)口光盤生產(chǎn)線設(shè)備類機電產(chǎn)品，須經(jīng)地方新聞出版主管部門批復(fù)（原件）且提交進(jìn)口用戶光盤行業(yè)經(jīng)營許可證（復(fù)印件）；

（5）進(jìn)口采用國際招標(biāo)方式采購的機電產(chǎn)品，應(yīng)提供國際招標(biāo)主管機構(gòu)簽發(fā)的《國際招標(biāo)評標(biāo)結(jié)果通知書》；

（6）進(jìn)口舊機電產(chǎn)品的，應(yīng)提供國家質(zhì)檢總局授權(quán)或許可的檢驗檢疫機構(gòu)出具的進(jìn)口的預(yù)檢驗報告；

（7）進(jìn)口超過一百萬人民幣未招標(biāo)的機電產(chǎn)品，應(yīng)提供注冊地工商局出具的私營企業(yè)證明，加蓋檔案查詢章或者公章；

（8）商務(wù)部規(guī)定的其他需要提交的材料。

（二）申請《重點舊機電產(chǎn)品進(jìn)口許可證》，應(yīng)當(dāng)提供以下材料：

1.申請進(jìn)口報告（介紹企業(yè)概況，擬申請進(jìn)口的舊機電產(chǎn)品的用途說明）；

1 apply for import report (Introduction to the general situation of the company, the intended use of the used mechanical and electrical products to be imported);

2.機電產(chǎn)品進(jìn)口申請表（將制造年限標(biāo)注在申請表格的規(guī)格型號欄內(nèi)并加蓋進(jìn)口用戶單位印章）；

2 Mechanical and electrical products import application form (to be marked in the application form of the term manufacturing specifications and affix the import user unit seal box);

3.進(jìn)口用戶的營業(yè)執(zhí)照復(fù)印件（需加蓋進(jìn)口用戶單位印章）； 3 copy of the business license of the import user (with the seal of the import unit);

4.申請進(jìn)口的重點舊機電產(chǎn)品的制造年限證明材料；

4 Certificate of manufacturing life of key used mechanical and electrical products for import;

5.申請進(jìn)口單位提供設(shè)備狀況說明；

5 apply for import equipment status;

6.屬于委托代理進(jìn)口的，應(yīng)當(dāng)提交委托代理進(jìn)口材料；

6 where the principal agent is imported, it shall submit the import materials of the principal agent;

7.其他相關(guān)法律、行政法規(guī)對“進(jìn)口用戶”或者“進(jìn)口商品用途”有特定規(guī)定的，應(yīng)提交“進(jìn)口用戶”或者“進(jìn)口商品用途”符合國家規(guī)定的相關(guān)證明材料。

申請企業(yè)應(yīng)提供國家相關(guān)部門核發(fā)的經(jīng)營許可證（復(fù)印件）。如：《印刷經(jīng)營許可證》、《水路運輸許可證》等。
