3、中文標簽樣張（1:1比例）

4、中英文對照配比表（另附電子版）

5、委托方營業(yè)執(zhí)照復印件（需蓋章）

6、獲獎、獎證及法定產(chǎn)區(qū)有關證明復印件及翻譯件（需蓋章）

7、原產(chǎn)地證復印件或產(chǎn)品在生產(chǎn)國（地區(qū)）允許生產(chǎn)、銷售的證明文件復印件（需蓋章）

二、進口食品商檢所需資料：（有表格樣板，填寫即可）

進口食品貨物清單、進口食品適用標準說明、進口食品包裝情況說明、進口食品安全承諾書

三、進口食品報關所需資料

國外供應商：原產(chǎn)地證、衛(wèi)生證、生產(chǎn)日期證明、提單、貿(mào)易合同、裝箱單、商業(yè)發(fā)票等。

Shipper: C/O ,Sanitary certificate,certificate of production date ，bill of lading, contract,packing list,invoice

國內(nèi)進口商：食品流通許可證、營業(yè)執(zhí)照、換單委托書、報檢委托書、報關委托書等

Consignee/importer：Food circulation permit, business license, in a single power of attorney, inspection declaration a power of attorney, customs a power of attorney
